I confirm receipt of 3 emails relating to this submission.

I draw your attention to the email from the Chairperson on 8 April (attached for ease of reference) – as we are working to a tight schedule, the later submissions are received, the greater the risk that the Inquiry will not have time to consider them.

Could you please pass this on to the appropriate person?

While I start on further Submissions, I think I should at least flag at least the Minimum I shall be lodging under

KEY ISSUE 6

The measures which ought to be considered or
investigated for the purpose of avoiding a similar serious accident occurring
in the future.

1) Structure of Mines Inspectorate and Competencies Required as Mines Inspector.

2) Oversight and Day to Day Management/control of RSHQ.

3) Right to Information

4) Investigations, Information shared with Industry and process involved

5)  Acceptance of The Expert Legal Assessment CMSHA, CMSHR and Recognised Standards (UQ report) and the resulting committee

6) ” Reasonably Foreseeable” “Acceptable Level of Risk”. Application of Grasstree Supreme and Appeal Court decisions 2005 and 2006. Wider adoption and definition of Reasonably Foreseeable in the Coal Mining Act and Regulations.

7) SIMTARS (Structure, Functions, Research, Conflict of Interest)

8) Unintended Consequences Changes to Act, including direct employment of Statutory Officials by the Coal Mine Operator, Industrial Manslaughter.

9) Unintended Consequences for changes to Methane Regulations

10) Changes to the Act for the Return of Mining Warden Inquiries

As well I have to prioritize Key Issue 4 numbers 1 and 2 and all the Key Issue 5 that I have not yet submitted, let alone tackle the other Key Issues

One other Submission you will likely receive today is on the need for more public hearings in, based on the information that I believe was not raised in previous Public hearings that is contained in the Interview of Adam Maggs.
