Mine Fatality Investigation Reports Deliberately Withheld from Public. RSHQ (DME) Cult of Secrecy

With the demise of the Mining Warden and mandatory Inquiries, there has been only a few of the 47 Mines Inspectorate Fatality Reports released even to the Relatives of the Deceased let alone for public scrutiny.

The Mining Warden was also the Coroner.

I wrote to the Chief Inspector requesting the same access to the 47 Fatality Reports that Dr. Sean Brady was given when he wrote the Qld Government commissioned

“Review of all fatal accidents in Queensland Mines and Quarries 2000 to 2019”

CIOCM Newman refers me to the Right for Information and closes the matter as far as Mines Inspectorate are concerned

To the Chief Inspector of Coal Mines; Qld DNRME/Whomever It May Concern

Re 47 DNRME Mine Fatality Investigation Reports

Dear Peter;

I am unsure if you are the correct person to address this formal request to. If not, could you pass it on to the person responsible.

I noted in the Government Commissioned Brady Report.

The author makes mention of being afforded the opportunity to read, study and analyze the 47 Reports.

Mr Brady has been afforded an opportunity to collectively review information that has never been afforded to any person or group before, and certainly not anyone outside the Mines Inspectorate.

I am not requesting access to any Fatalities where the Mines Inspectorate either has already initiated legal proceedings against or is still within the time frame for initiating proceedings.

On my estimation that will be just under 40 Reports.

I am formally requesting this information be made available to the public including myself.

To clear any doubt, please do not cite the criteria of being potentially affected by Coronial proceedings except regarding the Grasstree fatality.

Can you please advise me of the timeframe for a final response?

Yours In safety

Stuart Vaccaneo
