Whether the operational practices and management systems in existence at Grosvenor Mine, or at higher corporate levels, were apt to adequately respond to the mines understanding of the difficulties it was having with Longwall 104.
FINDING
1. The Anglo LFI Investigation Process Sign off Identifies the SSE (Site Senior Executive) as the General Manager.
ALL THE LFI’s tendered to the Grosvenor Inquiry have the Title General Manager for the SSE in the Investigation Report Acceptance Sign off.
NO POSITION of GENERAL MANAGER EXISTS UNDER THE CMSHA 1999
2. This the Second Instance where a Coal Mining Operator (Company) has identified or Appointed someone to the Position of General Manager.
The first being BMA as identified by the Mines Inspector Investigation into the Daniel Springer Fatality at Goonyella Riverside Mine which I have covered in Submissions.
3) Both the Grosvenor LW Face Ignition and the Daniel Springer Fatality involved contract production and engineering workers.
RECOMMENDATION
RSHQ MINE INSPECTORS TAKE IMMEDIATE ACTION TO ENSURE THE SITE SENIOR EXECUTIVE is THE MOST SENIOR MANAGEMENT POSITION APPOINTED BY THE OPERATOR at ALL QUEENSLAND COAL MINES.
