31st August 2016 Investigation June 6 Roof Fall continued

POINTS TO NOTE

  • Nearly 3 months has elapsed since the June 6th Fall

What was in the Mine Investigation report that prompted the Inspectorate to conduct interviews?

The Investigation is supposed to be provided to Inspectorate within one month.

Why has it taken a further 2 months?

 

  • Assume that means a member of Mine Management who wanted a joint interview with three persons.

Does this 3 include Inspector Gouldstone, or has the Management Person have 2 other people present?

Does that mean they wanted Legal Representation?

 

  • No indication when Inspector will conduct the Interviews with ERZC from previous shifts.

Did the Inspector ever Interview them and conclude his Investigation and findings?

 

  • Who is the Geotechnical Expert at the DNRME?

Do they have Coal Mining Competencies and demonstrable experience?

What is Inspector Goldstone’s competencies regarding coal geotechnical and strata support?

 

  • Was Investigation report provided within 30 days as required?

How many Reports are there?

What is the content of Reports?

Why is Inspector so confident that Terry Medhurst is an “Impartial Expert”. Is he sure there are no previous or current contract arrangements with Anglo?

 

  • “Provide Feedback” means little to nothing under the CMA 1999.

Suggested “document/presentation which gives the experience so far gleaned regarding strata control”

Surely that it what a proper Investigation into 3 major roof falls now would do?

Workers were trapped at the face and were fortunate to be able to escape in the first one on the 6th of June.

What more does it take to force action on the Grosvenor SSE via Directives?

 

CLOSE OUT MEETING

 

Inspector congratulates them, when for once the Mine and Officials actually cooperated as they are required to do under the Act. Why?  

  

FALL INVESTIGATION from 6/6/2016 CONTINUES with no end in sight.

2 major falls since will be considered when the DNRME concludes Investigation from 5th June.
