I heard that the Inspectors have been at Peak Downs very recently late last week looking at Dozer ops/SHMS,

Thanks you for the follow up. I have discussed this with my regional inspectors and a complaint concerning this event is currently  being investigated. I will wait until I have reviewed the outcomes of that investigation, and a planned visit to the site by myself in the next couple of months, before coming back to you.

I am just inquiring about when you will send the written response to my complaint.

Again I realise the continuing fatalities are a considerable drain on your time and resources.

As I stated, in my view only a Ministerial Board of Inquiry into the at least one of the recent fatalities is required.

The Houston Fatality at Saraji displays the deep seated issues that need to be examined in such a Board of Inquiry.
